Work Environment

This position is based onsite, primarily in the community and patients' homes, which allows for direct engagement with diverse populations. Our work environment is collaborative, utilizing various tools and systems to streamline care and enhance patient engagement. Our operating hours include weekdays from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m., and flexibility may be required for community outreach events during weekends.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, ensuring a complete understanding of their healthcare needs.
• Diagnose and manage both acute and chronic medical conditions, focusing on preventive care and health promotion strategies tailored to the community.
• Collaborate with local organizations and stakeholders to develop and implement impactful health education programs that directly address community health issues.
• Provide culturally sensitive care and advocate for patients' rights, ensuring access to necessary healthcare services for all individuals.
• Coordinate referrals and follow-up care to maintain continuity for patients, ensuring they receive the comprehensive care they require.
• Maintain detailed and accurate medical records in adherence to regulatory standards, facilitating seamless communication within the healthcare team.
• Actively participate in community outreach events and health fairs, offering health screenings and educational resources to raise awareness.
• Stay informed about community health trends, evidence-based practices, and relevant policies to continuously enhance care delivery.

Patient Population / Client Focus

This role primarily focuses on underserved populations in Mamaroneck and surrounding areas, many of whom may face barriers to accessing healthcare. You will work closely with diverse community members, tailoring your approach to meet the cultural and social needs of patients, ensuring that everyone receives equitable care.

Qualifications

• Current certification as a Family Nurse Practitioner is required.
• Active and unrestricted state nursing license to practice in New York.
• A Master’s degree in nursing from an accredited program is essential for this role.
• A minimum of 3 years of clinical experience, leading or coordinating health initiatives in community health or public health settings is required.
• Exceptional interpersonal and communication skills to engage effectively with diverse populations and work collaboratively with community partners.

Compensation and Benefits
