Work Environment

The position is based onsite at our modern hospital facility located in Harrison, NY. The typical workweek consists of approximately 40 hours, with shifts primarily scheduled between 9:00 a.m. and 6:00 p.m. Some evenings until 8:00 p.m. may be required, along with participation in community health fairs during weekends.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for individuals and families, employing both clinical skills and personal interaction.
• Diagnose and manage a range of acute and chronic medical conditions, ensuring adherence to best practices in preventive care.
• Develop and implement health education programs in collaboration with community organizations, focusing on preventive measures and wellness promotion.
• Provide advocacy and culturally competent care to patients from diverse backgrounds, ensuring they understand their rights within the healthcare system.
• Manage care transitions by coordinating referrals and follow-up services to guarantee continuity in patient care.
• Maintain meticulous medical records in accordance with regulatory standards, utilizing our electronic health record (EHR) system to enhance patient care delivery.
• Engage in community outreach initiatives and health fairs to provide health screenings and raise health awareness among residents.
• Stay informed about current community health trends, emerging healthcare practices, and pertinent policy changes.

Qualifications

• Current certification as a Nurse Practitioner.

Compensation and Benefits

Jackson Medical Center offers a competitive salary range of USD 180,000 to 200,000 per year, commensurate with experience and qualifications.
