Key Responsibilities

• Conduct thorough initial patient screenings, including evaluations of patient history and physical examinations, to assess health status and identify any immediate healthcare needs.
• Maintain accurate and up-to-date patient records at each visit, ensuring compliance with healthcare regulations and facilitating high-quality care.
• Analyze laboratory test results and diagnostic imaging to determine the necessity for follow-up visits and additional treatment options.
• Deliver consistent, high-quality care while meeting established productivity metrics and organizational goals.
• Serve as a trusted resource for patients seeking comprehensive care information, guidance on ongoing health conditions, and effective counseling for chronic illnesses.
• Educate patients and their families on disease prevention strategies, health maintenance practices, and treatment plans tailored to individual needs, with a focus on harm reduction techniques.

Patient Population / Client Focus

You will be working predominantly with an elderly demographic, including individuals managing chronic health conditions and those in need of preventive care services. The role involves understanding and addressing the unique healthcare challenges faced by seniors, promoting not only their physical health but also their emotional wellbeing. You will be integral in educating patients on lifestyle changes and assisting them in navigating their healthcare journeys.

Qualifications

• A valid Family Nurse Practitioner license in New York.
• Certification in Family Medicine with a focus on primary health care.
• A Master's degree from an accredited program designed to prepare registered nurses for advanced practice roles.
• Proficiency in performing comprehensive physical assessments, diagnosing common illnesses, and managing chronic diseases effectively.

Preferred Experience

Compensation and Benefits

We offer a competitive annual salary ranging from $150,000 to $160,000, commensurate with experience, along with a comprehensive benefits package that includes health insurance, retirement plans, and paid time off.
