Pulse Point Internal Medicine is seeking a dynamic and dedicated Nurse Practitioner to join our home care team in Harrison, New York. This full-time position allows you to practice independently while also collaborating with physicians to enhance patient care. With a competitive salary range of $180,000 to $200,000 annually, the role emphasizes providing high-quality, value-based primary care to Medicare-eligible adults.

Work Environment

As a Nurse Practitioner, you will work within a collaborative care team that includes a physician, a nurse practitioner or physician assistant, medical assistants, a dedicated medical scribe, and care managers. This multidisciplinary team approach allows for comprehensive support as you focus on primary care and care coordination while partnering with a network of elite specialists and hospitals.

Key Responsibilities

• Provide exceptional primary care during office visits, addressing routine and acute health concerns.
• Administer annual wellness visits and health risk assessments, placing a strong emphasis on holistic health care and accurate documentation.
• Coordinate care with other health care providers, specialists, and testing facilities to optimize patient treatment plans.
• Lead population health initiatives, ensuring all eligible patients receive required screenings, like evidence-based breast cancer screening.
• Assist the care team with phone triage, patient outreach, and follow-up care efforts to enhance patient engagement.
• Educate patients on their medical conditions and treatment options, empowering them to make informed health choices.
• Conduct home visits as needed to provide continuity of care and support for patients with mobility challenges.
• Participate in promotional activities and community outreach to promote our patient services and care philosophy.
• Report directly to the Medical Director and collaborate closely with operational leadership to meet organizational goals.

Patient Population / Client Focus

This role primarily focuses on older adults enrolled in Medicare, emphasizing the importance of maintaining their health and enhancing their quality of life. Our care model is designed to meet the specific needs of this population, allowing practitioners to build deep, trusting relationships with patients and their families, resulting in better health outcomes.

Qualifications

• A Master of Science in Nursing, Doctor of Nursing Practice, or equivalent NP program graduate.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Active, non-probationary state Nurse Practitioner license and DEA license.
• Experience in primary care settings, specifically in internal medicine, geriatrics, or family medicine.
• Strong understanding of HCC documentation, ICD-10 coding, and health risk assessments.
• Passion for teamwork, collaboration, and a commitment to an innovative care model focused on population health.
• Bilingual proficiency is a plus.

Preferred Experience

• Previous experience in home care settings, providing in-home assessments and care.
• A familiarity with electronic health record (EHR) systems, particularly those tailored to outpatient settings.
• Demonstrated ability to work effectively in a cross-functional team.

Compensation and Benefits
