Senior Nurse Practitioner - Outpatient Care at Elderly Care & Rehab Clinic

Position Overview

We are seeking a dedicated and experienced Senior Nurse Practitioner to join our team at the Elderly Care & Rehab Clinic located in Yonkers, New York. This full-time, onsite position offers an exciting opportunity to provide high-quality primary care to a diverse population of older adults enrolled in Medicare. Work Environment

This role will take place in a dynamic outpatient setting within a collaborative healthcare team that includes physicians, nurse practitioners, physician assistants, medical assistants, and dedicated support staff such as medical scribes and care managers. The clinic environment promotes teamwork and communication, enabling providers to focus on what they do best: delivering exceptional primary care. Our typical working hours are Monday through Friday, 9:00 AM to 5:00 PM, allowing for a balanced work-life commitment.

Key Responsibilities

As a Senior Nurse Practitioner, you will be responsible for:

• Providing exceptional and personalized primary care to patients of varying complexities and needs.
• Conducting thorough office visits for both routine check-ups and acute health concerns, ensuring each assessment involves a comprehensive understanding of each patient's history and health status.
• Executing annual wellness visits and health risk assessments to capture a holistic view of each patient’s health, emphasizing meticulous documentation practices.
• Coordinating care with other healthcare providers, specialists, testing facilities, and community agencies to ensure continuity and effectiveness of care.
• Leading population health initiatives in collaboration with the Care Team, ensuring timely screenings such as mammograms for eligible patients.
• Assisting the care team with phone triage, patient outreach, and follow-up care.
• Educating patients and their families about health conditions, treatment plans, and preventive measures to enhance their understanding and engagement.
• Participating in community outreach and promotional activities to strengthen our patient education and engagement efforts.
• Conducting home visits for patients who require care in their own environment and for those who are homebound.
• Reporting directly to the Medical Director and closely collaborating with operational leadership for optimal care delivery.

Patient Population / Client Focus

Our main focus is on older adults, primarily those enrolled in Medicare, who often present with a wide array of chronic conditions and health challenges. The nurse practitioner plays a critical role in providing longitudinal care, building trust with patients, and ensuring they understand and feel connected to their personal care plans. By tailoring our service delivery to the unique needs of our patient population, we strive to improve overall health outcomes and enhance the quality of life for the seniors we serve.

Qualifications

The ideal candidate must possess:

• A Master of Science in Nursing (MSN), a Doctor of Nursing Practice (DNP), or completion of a Nurse Practitioner Certificate Program.
• National certification as a Family Nurse Practitioner, Adult Nurse Practitioner, or Adult-Gerontology Primary Care Nurse Practitioner through recognized bodies such as AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license and DEA license.
• Previous experience in primary care settings, especially internal medicine, geriatrics, or family medicine.
• Expertise in HCC (Hierarchical Condition Categories) documentation and ICD-10 coding, coupled with a solid understanding of Health Risk Assessments (HRAs).
• A strong affinity for teamwork and a collaborative spirit suited for a cross-functional environment.

Preferred Experience

Additional desirable qualifications include:

• Experience in telehealth or remote patient management systems that aid in delivering care to homebound or underserved populations.
• Knowledge of community resources to integrate patients into local health systems effectively.
• Familiarity with EMRs and practice management software, ensuring efficient documentation and patient management capabilities.

Compensation and Benefits

We offer a competitive salary ranging from USD 120,000 to 160,000 per year, commensurate with experience and qualifications.
