Work Environment

This position is based in our Yonkers outpatient office, where you will engage with patients in a friendly and professional atmosphere. You will have the opportunity to work in a team-oriented environment, closely collaborating with physicians, nurses, and other healthcare providers to develop and implement patient care plans.

Key Responsibilities

• Assess and diagnose patient health conditions through comprehensive physical exams, reviewing medical histories, and ordering necessary diagnostic tests.
• Develop and implement patient-centered treatment plans in collaboration with other healthcare professionals.
• Provide patient education regarding health promotion and disease prevention strategies tailored to individual patient needs.
• Manage and treat acute and chronic health conditions, monitoring patient progress and adjusting treatment plans as necessary.
• Document patient care activities accurately using our electronic health record (EHR) system, ensuring compliance with regulatory standards.
• Participate in quality improvement initiatives aimed at enhancing clinic operations and patient satisfaction.

Qualifications

• Master's degree in Nursing from an accredited institution.
• Current Nurse Practitioner certification and active state nursing license in New York.
• Minimum of 3 years of clinical experience, particularly in community health or public health settings.
• Strong interpersonal and communication skills, effectively engaging with patients from diverse backgrounds.
• Commitment to health equity and advocacy for vulnerable communities.

Preferred Experience

• Experience in outpatient clinical settings with a focus on chronic disease management.
• Familiarity with electronic health record (EHR) systems to streamline patient documentation and care.

Compensation and Benefits

We are proud to offer a competitive annual salary ranging from $150,000 to $180,000.
