Experienced Nurse Practitioner - Cardiology Focused

Position Overview

The Integrative Health and Wellness Clinic is actively searching for a dedicated Nurse Practitioner (NP) to join our dynamic team at our specialized Cardiology Clinic in the Bronx, New York. This full-time, onsite position will support our outpatient practice at the Center for Advanced Cardiac Care (CACC). The NP will work under the leadership of the Chief Nurse Practitioner and the guidance of the CACC Medical Director, contributing to an interdisciplinary approach to patient care in cardiology.

As a Nurse Practitioner in the Bronx, you will be immersed in a collegial environment within our school-based facility designed to foster collaboration and communication among healthcare professionals. The CACC team includes physicians, fellow NPs, registered nurses, social workers, nutritionists, and various specialists committed to providing holistic care. The role demands a blend of autonomy and teamwork as you balance independent decision-making with collaborative patient management.

Key Responsibilities

The Nurse Practitioner will undertake a variety of clinical responsibilities, which include but are not limited to:

• Gathering comprehensive and targeted patient histories and conducting physical examinations to assess both acute and chronic health conditions.
• Ordering and interpreting diagnostic tests such as EKGs, echocardiograms, and blood tests, followed by managing treatment plans tailored to individual patient needs.
• Developing goal-directed healthcare plans involving preventive care, lifestyle management, and timely medical interventions per established clinical protocols.
• Providing follow-up care and collaborating intensively with attending physicians to modify treatment plans based on patient progress and feedback.
• Educating patients and their families about cardiac health, potential disease processes, and effective self-management strategies outside the clinical setting.
• Coordinating admissions and discharges, ensuring seamless transition between outpatient and inpatient care settings, and participating in the clinical decision-making process with the inpatient team.
• Triage daily patient inquiries, facilitating effective communication with referring providers and managing any urgent care needs.
• Documenting all patient interactions in our electronic medical records system accurately and in real-time.
• Engaging in research efforts to enhance clinical practice, including reviewing the latest cardiology findings and implementing evidence-based protocols.
• Promoting a patient-focused and therapeutic atmosphere that prioritizes safety and quality of care.

Patient Population / Client Focus

This position will focus on managing a diverse patient population, including adults with chronic cardiac conditions such as heart failure, pulmonary arterial hypertension, and those post-heart transplant. The NP will also support patients undergoing advanced therapies, including intravenous or inhaled medications and mechanical circulatory support. The ideal candidate will demonstrate compassion and dedicated support to optimize patient outcomes and improve their quality of life.

Qualifications

To thrive in this role, candidates must possess the following:

• A Master's Degree in Nursing with a specialization in Nurse Practitioner training oriented towards adult cardiac care.
• A minimum of two years of clinical experience, ideally within a hospital or outpatient cardiac care setting.
• Current New York State RN and NP licenses coupled with board certification as a Nurse Practitioner.
• Additional certifications, including Basic Life Support (BCLS), Advanced Cardiovascular Life Support (ACLS), DEA registration, and a National Provider Identifier (NPI).
• Proficiency with electronic medical record systems and standard office software packages, including Microsoft Word and Excel.

Preferred Experience

While the above qualifications are essential, candidates with experience in managing complex cardiac cases, such as those involving heart failure or patients undergoing advanced therapies, will be given preference. A demonstrated ability to work collaboratively in a team-oriented environment is also considered an asset.

Compensation and Benefits

The Integrative Health and Wellness Clinic offers a competitive salary range of USD 120,000 – 220,000 annually, commensurate with experience and qualifications.
