Work Environment

This position will be based onsite at our Jackson Heights facility. You will work in an innovative and collaborative environment where the emphasis is placed on patient independence and comprehensive care delivery. Your work schedule will typically span from 9:00 a.m. to 6:00 p.m., with potential occasional evening clinics running until 8:00 p.m. Weekend involvement in community outreach events or health fairs may also be required, averaging around 40 hours per week.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, aiming to identify both acute and chronic medical conditions.
• Develop and implement patient-centered care plans that include diagnostics, management strategies, and preventive measures.
• Work closely with community organizations to design and execute health education campaigns that target prevalent health issues in the area.
• Ensure culturally sensitive approaches in all aspects of care, advocating fiercely for patients’ rights and access to necessary services.
• Facilitate referrals and coordinate follow-up care with other healthcare professionals and services, ensuring continuity and comprehensive care for patients.
• Document patient interactions and care plans accurately in compliance with all regulatory standards to maintain high-quality records.
• Engage in community outreach efforts and health screenings, enhancing public awareness regarding health maintenance and disease prevention.
• Stay informed about emerging community health trends and adapt practices to align with evidence-based medicine.

Patient Population / Client Focus

Your primary focus will be on serving individuals and families from a wide range of cultural backgrounds in Jackson Heights. The community is known for its diversity, and you will be required to navigate various cultural perspectives on health and wellness. Providing equitable healthcare access to underserved and vulnerable populations will be paramount in this role, making your contribution vital for improving overall health outcomes.

Qualifications

• Must hold a current certification as a Nurse Practitioner and possess an active state nursing license.
• A Master’s degree in Nursing from an accredited program is required.
• Demonstrated experience with a minimum of 3 to 5 years in clinical settings, preferably in community health or public health environments.
• Excellent interpersonal and communication skills tailored for diverse populations.
• A strong commitment to social justice, health equity, and the advocacy of vulnerable communities.

Preferred Experience

Experience in concierge medicine or a similar setting is advantageous but not required. Familiarity with electronic health records (EHR) systems and community health initiatives will set applicants apart. Your background in implementing public health strategies and engaging with local organizations for outreach efforts will be highly regarded.

Compensation and Benefits
