Key Responsibilities

• Partner with a team of healthcare professionals to deliver comprehensive medical care to assigned patients across various stages of health and wellness.
• Conduct thorough assessments by obtaining detailed medical, physical, and psychosocial histories, performing physical examinations tailored to each patient's age and health status.
• Administer preventive screening procedures and develop differential diagnoses based on patient history and clinical findings.
• Create personalized patient education plans that empower patients to manage their health effectively.
• Prescribe and manage appropriate pharmacological and non-pharmacological treatments in accordance with established protocols.

Patient Population / Client Focus

This position involves caring primarily for residents in an assisted living facility, encompassing a range of health conditions typical of older adults, including chronic illnesses and conditions requiring regular monitoring. You will focus on maintaining and improving the quality of life for these individuals through preventive care, disease management, and health education.

Qualifications

• A Master’s Degree in Nursing with a specialization as a Nurse Practitioner.
• An active Registered Nurse (RN) license in New York State.
• Minimum of 3 years of clinical experience in a relevant healthcare setting, particularly in internal medicine or geriatrics.
• Proficient in using electronic health record (EHR) systems and other medical technologies to enhance care delivery.
• Exceptional patient education skills, advocating for patient needs and fostering health literacy.
• Strong diagnostic abilities with experience in formulating and implementing effective treatment plans.

Preferred Experience

• Experience in an assisted living or long-term care facility is highly desirable.

Compensation and Benefits

This position offers a competitive salary range of USD 150,000 – 160,000 per year, reflecting your experience and expertise.
