Work Environment

This role is set in a dynamic, onsite outpatient office located in Valley Stream, New York. Our facility is equipped with modern technology and resources, allowing for efficient patient care and electronic medical record (EMR) documentation. Key Responsibilities

• Conduct comprehensive assessments of patients, including documentation in our EMR system to ensure compliance with health records standards.
• Diagnose and manage acute and chronic health conditions, providing evidence-based treatment plans.
• Provide thorough health education and counseling to patients, families, and caregivers for effective disease management and health maintenance.
• Collaborate with primary care providers (PCPs) and an interdisciplinary care team to coordinate comprehensive care tailored to the unique needs of each patient.
• Participate in family conferences to enhance communication and understanding around advanced care planning.
• Conduct monthly, quarterly, and annual regulatory visits to monitor patient progress and ensure adherence to care protocols.
• Utilize telemedicine and other technology to connect patients with specialists, ensuring holistic care strategies.
• Address and evaluate pharmacy recommendations, ensuring safe medication management for our residents.
• Work onsite during designated hours to maintain sufficient patient coverage and foster a robust care environment.

Patient Population / Client Focus

The primary patient population served by our Nurse Practitioners includes elderly individuals and low-income adults who often face complex medical challenges. Our NPs are dedicated to improving health outcomes for these groups through personalized care strategies, enabling patients to manage their conditions effectively and enhance their quality of life.

Qualifications

• A Master's Degree in Nursing, Doctor of Nursing Practice, or completion of an NP Certificate Program is required.
• Current and unrestricted state license to practice as a Nurse Practitioner.
• Active DEA license and US work authorization are mandatory.
• National board certification from either AANP or ANCC.
• Proficiency in utilizing EMR systems for accurate record-keeping.
• Exceptional communication skills, capable of interacting effectively with patients, families, and healthcare team members.
• A strong commitment to providing care to senior and low-income populations.
• Flexibility to adapt to changing schedules and patient needs.

Preferred Experience

• Previous experience in an outpatient or primary care setting is highly desirable.
• Experience working with elderly patients and the complexities of geriatric medicine.
• Familiarity with integrated care models and value-based care initiatives will be beneficial.

Compensation and Benefits

We offer a competitive salary range of $150,000 to $160,000 annually, commensurate with experience and qualifications. Our comprehensive benefits package includes:
