Work Environment

Your role as a Nurse Practitioner will occur in a collaborative and supportive environment within the passionate team at Care Point. The work setting is primarily home care, where you will directly interact with patients in various community settings, including their homes and during community health outreach events. Our typical work schedule ranges from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in weekend community outreach events and health fairs is essential, with an average commitment of around 40 hours a week.

Key Responsibilities

• Conduct comprehensive health assessments to evaluate both physical and mental health, actively engaging with patients to gather necessary information for effective diagnosis and treatment planning.
• Diagnose and manage acute and chronic medical conditions, tailoring preventive care and health promotion strategies to individual patient needs.
• Engage in collaborative efforts with local community organizations and healthcare stakeholders to develop and implement educational health programs that enhance community awareness.
• Foster culturally sensitive healthcare practices by advocating for patients’ rights and ensuring equitable access to healthcare services for underserved populations.
• Coordinate referrals and follow-up care, ensuring that patients receive comprehensive support throughout their healthcare journey to facilitate continuity of care.
• Maintain meticulous and up-to-date medical records, in compliance with clinical guidelines and state regulations, ensuring privacy and accuracy in patient documentation.
• Participate actively in community outreach events and health fairs, promoting health education and offering essential screenings to raise health awareness among residents.
• Stay informed about community health trends and best practices in care delivery to continuously improve patient outcomes.
