Key Responsibilities

• Conduct thorough health assessments and screenings, taking a detailed medical history and evaluating patients' physical health and social factors impacting their well-being.
• Diagnose and manage patients with both acute and chronic medical conditions, formulating treatment plans that include preventive measures and health promotion strategies.
• Work hand-in-hand with community organizations and stakeholders to devise and promote health education programs tailored to the needs of the local population.
• Advocate for patients' rights, ensuring they have access to necessary healthcare services, and provide culturally sensitive and inclusive care.
• Coordinate and manage follow-up care, including referrals to specialists, to promote continuity of care and successful patient outcomes.
• Maintain accurate and detailed medical records in accordance with healthcare regulations and best practices.
• Participate in community outreach activities and health fairs, increasing awareness and access to healthcare screenings and services.
• Stay informed on current community health trends, evidence-based practices, and legislative policies that may impact patient care and public health initiatives.

Qualifications

• Current Nurse Practitioner certification and an active New York state nursing license.
• A Master's degree in Nursing from an accredited program is required.
• A minimum of three years of clinical experience, particularly in community health, public health, or a similar setting.

Compensation and Benefits

This full-time Nurse Practitioner position offers a competitive salary in the range of $180,000 to $200,000 annually, commensurate with experience and qualifications.
