Heal Well Medical Associates P.C. is actively seeking a dedicated and compassionate Family Nurse Practitioner (FNP) for a full-time role based onsite in a school setting in Long Beach, New York. Work Environment

This role is located onsite in a school, allowing you to engage directly with children, adolescents, and families. The work schedule is designed to accommodate community needs, typically ranging from 9:00 a.m. to 6:00 p.m., with occasional evening clinics available until 8:00 p.m. Participation in weekend community outreach events and health fairs is encouraged to extend services and enhance community health awareness. The anticipated workload is approximately 40 hours per week, allowing for a balanced approach to both professional responsibilities and community involvement.

Key Responsibilities

• Conduct thorough health assessments and screenings, ensuring a comprehensive understanding of patients’ medical histories.
• Diagnose, treat, and manage both acute and chronic medical conditions, emphasizing preventive care and wellness education.
• Collaborate effectively with community organizations to develop and implement health education programs that address pressing public health concerns.
• Provide advocacy for patients’ rights and enhance their understanding of available healthcare services.
• Coordinate referrals and follow-ups, ensuring seamless transitions in care and adherence to treatment plans.
• Maintain detailed and accurate medical records in compliance with healthcare regulations and internal standards.
• Engage actively in community outreach initiatives to promote health and wellness and improve health literacy.
• Stay informed about current health trends, evidence-based practices, and changes in healthcare policies impacting patient care.

Qualifications

Preferred Experience

Compensation and Benefits

This position offers a competitive salary range of USD 180,000 – 200,000 per year, commensurate with your experience and qualifications.
