Work Environment

This role is primarily onsite, allowing for direct patient interaction and immediate response to health concerns. The usual work schedule is Monday through Friday, from 9:00 a.m. to 6:00 p.m., with potential evening clinics and weekend community outreach opportunities. Key Responsibilities

• Conduct thorough health assessments and screenings for diverse patient populations, employing diagnostic tools and methodologies tailored to individual needs.
• Diagnose acute and chronic medical conditions, offering evidence-based treatment plans that include preventive care and health promotion strategies.
• Develop and implement community health education programs in collaboration with local stakeholders, focusing on crucial health topics relevant to our patient population.
• Maintain accurate and detailed medical records using our digital health record systems, ensuring compliance with regulatory standards and facilitating continuity of care.
• Actively participate in organizing and providing health screenings at community fairs, enhancing awareness about available services and healthcare resources.
• Advocate for patients’ rights, helping them navigate complex healthcare landscapes to secure necessary services.
• Stay informed about the latest community health trends and participate in training sessions to ensure the integration of evidence-based practices in patient care.

Patient Population / Client Focus

Our service population includes individuals and families from various backgrounds in Long Beach and surrounding areas, with a diverse array of healthcare needs. Qualifications

• Current certification as a Family Nurse Practitioner from a recognized program.
• Active nursing license in the State of New York.
• A Bachelor’s degree in Nursing, complemented by a Master’s degree in Nursing from an accredited institution.
• Minimum of 3-5 years of clinical practice experience, ideally in community health or public health settings.
• Proficient interpersonal and communication skills, with a demonstrated ability to engage effectively with diverse populations.
• Commitment to equity in healthcare and passionate about advocating for socially vulnerable communities.

Preferred Experience

• Experience working within a concierge medicine or house-call practice model.
• Familiarity with health education program development and implementation.
• Previous involvement in community health initiatives or outreach programs.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 annually, commensurate with experience and qualifications.
