Blissful Nurse Specialists is seeking a dedicated and compassionate Family Nurse Practitioner (FNP) to join our team, providing comprehensive, onsite healthcare services in Rockville Centre, New York. You will engage with patients in a concierge medicine setting, performing house calls to ensure that care is accessible, personalized, and holistic.

Work Environment

The ideal candidate will work directly in the community, conducting house calls to provide care that aligns with our philosophy of concierge medicine—where personalized service, accessibility, and a focus on the individual are paramount. Your typical working hours will be from 9:00 a.m. to 6:00 p.m., with occasional evening service until 8:00 p.m. Participation in weekend community outreach programs may also be required, emphasizing our organization's commitment to proactive health education.

Key Responsibilities

• Conduct detailed health assessments tailored to the unique needs of individuals and families, initiating a health history and physical examinations during house calls.
• Diagnose and manage acute and chronic medical conditions, and develop individualized care plans that include preventive measures and health promotion strategies.
• Collaborate with local organizations to design and implement community health education programs, enhancing awareness of preventable diseases and wellness practices.
• Provide culturally competent care, actively advocating for patients' rights and facilitating access to necessary healthcare services.
• Coordinate follow-up care by managing referrals, ensuring smooth transitions between specialists, and maintaining continuity of care through diligent record-keeping.
• Document patient assessments, treatment plans, and progress notes in an electronic medical record (EMR) system, ensuring compliance with legal and ethical standards.
• Participate in community health fairs or outreach events to promote health and well-being, offering screenings and educational resources.
• Remain informed of the latest community health trends, legislation, and evidence-based practices to ensure the highest standard of care.

Patient Population / Client Focus

The Family Nurse Practitioner will serve a diverse client population, ranging from young families to elderly individuals in Rockville Centre and its vicinity. Qualifications

• A current and valid Nurse Practitioner certification with an active New York state nursing license.
• Completion of a Master’s degree in nursing from an accredited institution.

Compensation and Benefits

We offer a competitive salary in the range of USD 180,000 to 200,000 per year, commensurate with experience and qualifications.
