Work Environment

As a Nurse Practitioner in our clinic, you will work onsite within our collaborative care team, which includes physicians, fellow Nurse Practitioners and Physician Assistants, medical assistants, and care coordinators. You will have access to extensive support, including a dedicated medical scribe, registered nurses, and care managers, allowing you to focus on patient care without the burdens of administrative tasks. Key Responsibilities

• Provide exceptional and patient-centered primary care tailored to individual patient needs.
• Conduct thorough office visits addressing both routine and acute health issues, ensuring comprehensive assessments and continuity of care.
• Administer annual wellness visits and health risk assessments, with a detailed approach to documentation that captures the holistic view of health for every patient.
• Coordinate care effectively with other healthcare providers, specialists, testing facilities, and agencies to ensure seamless patient journeys.
• Lead population health initiatives in partnership with care team members, proactively addressing preventive screenings and healthcare goals.
• Assist in phone triage and outreach efforts to enhance patient engagement and care management.
• Educate patients and their families on health conditions, care plans, and treatment options, promoting understanding and adherence to health strategies.
• Participate in community outreach and promotional activities to raise awareness about our services and engage potential patients.
• Conduct home visits when necessary to provide care for patients unable to visit the clinic.
• Report directly to the Medical Director, closely collaborating with operational leadership on clinic initiatives.

Patient Population / Client Focus

Your primary focus will be on older adults enrolled in Medicare, necessitating a keen understanding of their unique healthcare needs. The clinic's environment emphasizes building long-term, trusting relationships with patients, which is essential for understanding their individual circumstances and tailoring care plans that enhance their quality of life.

Qualifications

• A Bachelor’s Degree in Nursing is required; a Master of Science in Nursing or a Doctor of Nursing Practice is preferred.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner from recognized bodies such as AANP or ANCC.
• Possession of an active, non-probationary state Nurse Practitioner license and an active DEA license.
• Demonstrated experience in primary care focusing on internal medicine, geriatrics, or family medicine.
• Knowledgeable in HCC documentation, ICD-10 coding, and health risk assessments.
• Exceptional teamwork skills with a strong desire to collaborate across disciplines and contribute to a positive workplace culture.

Preferred Experience

• Experience in concierge medicine or house call practices is highly valued.
• Demonstrated proficiency in holistic health assessment and management.
• Previous experience working with a geriatric patient population is preferred.

Compensation and Benefits

This full-time position offers a competitive salary range of USD 120,000 to 160,000 annually, commensurate with experience and qualifications.
