Work Environment

This position is distinctively set within the framework of house calls, where NPs engage directly with patients in their homes. Key Responsibilities

• Conduct thorough physical examinations and medical assessments of patients, utilizing diagnostic tools and clinical expertise to form accurate health profiles.
• Design and implement individualized patient management plans that address acute and chronic conditions, ensuring continuity of care and improving health outcomes.
• Coordinate referrals to specialists and additional healthcare services, ensuring all aspects of patient care are effectively communicated and managed.
• Collaborate closely with supervising physicians and other healthcare providers to strategize patient care, exchange insights, and optimize treatment plans.
• Participate in multidisciplinary team meetings, presenting patient cases and discussing approaches to enhance overall care strategies.
• Stay updated on the latest medical research and treatment options to implement evidence-based practices in patient care.

Qualifications
