Work Environment

This role is primarily onsite, allowing our Nurse Practitioners to provide home-based care within the Hempstead area. Key Responsibilities

• Conduct comprehensive assessments and diagnostic evaluations of patients, addressing both acute and chronic medical conditions.
• Formulate and implement individualized treatment plans, leveraging clinical evidence and best practices to enhance patient care.
• Prescribe appropriate medications, therapies, and medical interventions, ensuring that treatment aligns with current healthcare regulations.
• Provide health education to patients and families, focusing on disease prevention, symptom management, and health promotion strategies relevant to home care.
• Collaborate proactively with healthcare professionals to coordinate patient care effectively, leading multidisciplinary meetings to discuss ongoing patient needs.
• Maintain accurate medical records utilizing our electronic health record (EHR) systems while ensuring compliance with HIPAA standards.
• Engage in continuous quality improvement initiatives that support the organization’s mission and enhance patient care processes.

Patient Population / Client Focus

The Nurse Practitioner will serve a diverse patient population, primarily composed of individuals requiring home care support due to chronic illnesses, recovery from surgeries, or disabilities. Qualifications

• Current certification as a Nurse Practitioner (NP) and an active state nursing license in New York.

Compensation and Benefits

The position offers a competitive annual salary ranging from USD 150,000 to 160,000 , commensurate with experience and qualifications. Additionally, we provide a comprehensive benefits package, including medical, dental, and vision insurance, a retirement plan, and generous paid time off.
