Key Responsibilities

• Conduct thorough assessments to determine patients' acute and chronic medical conditions, ensuring that each case is approached with a comprehensive understanding.
• Develop and implement evidence-based treatment plans that are collaboratively designed with other healthcare professionals, ensuring optimal care management.
• Order and interpret diagnostic tests, as well as prescribe necessary medications and therapies, while monitoring patient responses and adjusting treatments as needed.
• Educate patients and their families about disease management, medication adherence, and strategies for post-discharge care to promote long-term health benefits.
• Participate in regular interdisciplinary meetings to discuss patient progress, address concerns, and enhance care coordination among healthcare providers.

Patient Population / Client Focus

You will primarily work with a diverse patient population, ranging from individuals with chronic illnesses requiring ongoing management to those recovering from acute health issues. Qualifications

• Current certification as a Nurse Practitioner, with an active state nursing license in New York.
• A Bachelor’s degree in nursing is required, while a Master's degree is preferred to enhance clinical competencies and care quality.
• A minimum of 4 years of clinical experience as a Nurse Practitioner, ideally within home care or community health settings.

Compensation and Benefits
