Key Responsibilities

• Conduct thorough initial patient screenings including comprehensive health history assessments and physical examinations.
• Maintain and update accurate electronic patient records in accordance with HIPAA guidelines, checking for consistency and completeness during each appointment.
• Interpret laboratory results and diagnostic imaging to determine the need for further treatment or follow-up appointments, ensuring timely and appropriate care for patients.
• Deliver high-quality healthcare while managing productivity metrics, providing efficient services without compromising care quality.
• Act as a vital resource for patients by offering thorough information, guidance, and counseling on ongoing care, particularly for those with complex health issues.
• Provide patient education on health maintenance, illness prevention, and treatment plans, focusing on comprehensive harm reduction strategies.

Patient Population / Client Focus

Your role will encompass caring for a broad spectrum of patients, including families, children, and elderly individuals throughout the Hempstead area. Many of our patients are dealing with chronic conditions requiring ongoing management, and your support will be crucial to their health outcomes. You will have the opportunity to build long-term relationships with your patients, advocating for their health and wellness in their everyday lives while navigating the sometimes complex healthcare landscape.

Qualifications

• A Master’s degree in Nursing from an accredited institution.
• A current New York State licensure as a Nurse Practitioner (NP).
• A minimum of 2-3 years of experience as a licensed Nurse Practitioner, preferably in a family medicine or home care setting.
• Proficient communication skills for effective interaction with patients and the ability to manage sensitive medical situations compassionately.

Preferred Experience

• Prior experience working in a family practice clinic or caring for a pediatric and geriatric patient population.
• Familiarity with electronic health record systems and telehealth services to enhance patient care delivery.
• Current certifications in advanced practice nursing specialties or related fields would be considered an advantage.

Compensation and Benefits
