Work Environment

The Nurse Practitioner will work onsite at our facility located within a school in West Hempstead. This dynamic environment provides an opportunity to interact with students, families, and educational staff, allowing for a comprehensive understanding of community health needs. The work schedule is primarily Monday through Friday from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m., and participation in community outreach events on weekends.

Key Responsibilities

• Conduct thorough health assessments for individuals and families, incorporating preventive care strategies and health screenings.
• Diagnose and manage both acute and chronic medical conditions, tailoring care plans to meet the diverse needs of our patient population.
• Engage with community organizations to develop, implement, and evaluate health education programs aimed at improving overall health awareness.
• Provide culturally sensitive and patient-centered care, advocating for patients' rights and ensuring accessibility to necessary healthcare services.
• Facilitate referrals and aftercare processes, guaranteeing continuity of care to enhance treatment outcomes.
• Document patient information meticulously, adhering to the highest standards of regulatory compliance.
• Participate actively in community outreach efforts and health fairs, offering vital health screenings and education.

Patient Population / Client Focus

This position will primarily serve school-age children, adolescents, and their families, providing tailored healthcare that addresses their specific needs and challenges. Qualifications

• Current certification as a Nurse Practitioner.
• Active state nursing license in New York.
• Bachelor's degree in nursing; a Master's degree is highly preferred.
• Minimum of 3-5 years of clinical experience, particularly in community or public health settings.
• Strong interpersonal and communication skills, with the ability to engage effectively with diverse population groups.

Preferred Experience

Compensation and Benefits

The annual salary for this role ranges from USD 120,000 to 220,000, reflecting the candidate's qualifications and experience.
