Work Environment

This role will take place in a home care setting within Queens Village, NY, serving a diverse patient population. You will work in a vibrant environment characterized by teamwork, innovation, and social responsibility. Our typical work schedule starts from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in weekend community outreach events and health fairs may also be necessary, reflecting our commitment to health promotion beyond the clinic.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families to understand their health status and needs.
• Diagnose and manage a variety of health conditions, ensuring preventive care and health promotion techniques are employed effectively.
• Collaborate with community organizations to design and implement health education programs that address prevalent health issues and promote wellness.
• Provide culturally sensitive care, ensuring that patients’ rights and access to healthcare services are upheld.
• Coordinate patient referrals and follow-up care, facilitating smooth transitions and continuity of care among healthcare providers.
• Maintain accurate and detailed patient medical records in compliance with healthcare regulations and organizational standards.

Qualifications

• Current certification as a Nurse Practitioner, with an active state nursing license to practice in New York.
• A Master’s degree in nursing from an accredited program is required.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 annually, commensurate with experience and qualifications.
