Work Environment

This position is onsite, based in our hospital facility in Queens Village. Our clinic operates primarily during weekdays from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending to 8:00 p.m. The team may also participate in outreach events and health fairs on weekends. Your average weekly commitment will be approximately 40 hours, allowing for a balance of clinical duties and community engagement.

Key Responsibilities

• Perform thorough health assessments and screenings, ensuring comprehensive understanding of patients' health histories.
• Diagnose and manage a wide array of medical conditions, tailoring preventive care and health promotion strategies to individual needs.
• Leverage partnerships with community organizations to design and implement impactful health education programs that resonate with diverse populations.
• Ensure culturally sensitive care, advocating vigorously for patients' rights and healthcare access in alignment with community values.
• Manage referrals and follow-ups effectively to facilitate ongoing care and maintain continuity in patients' healthcare journeys.
• Document all patient interactions meticulously while adhering to HIPAA regulations and maintain accurate medical records.
• Engage actively in community health initiatives, including health screenings during local events, to raise awareness and foster engagement.
• Keep abreast of current community health trends, evidence-based practices, and relevant healthcare policies that impact our service delivery.

Patient Population / Client Focus

As a Family Nurse Practitioner at Radiant Wellness Clinic, you will primarily serve a diverse patient population that includes individuals and families from various cultural and socioeconomic backgrounds. Qualifications

• A current certification as a Family Nurse Practitioner is required.
• Active state nursing license in New York is essential for this position.

Compensation and Benefits

This position offers a competitive salary ranging from $180,000 to $200,000 annually, commensurate with qualifications and experience.
