Work Environment

Located within a supportive hospital environment in Glen Cove, our facility offers a dynamic workspace where teamwork and collaboration thrive. The Family Nurse Practitioner will engage in a structured yet flexible work schedule primarily from 9:00 a.m. to 6:00 p.m., with possibilities for occasional evening clinics extending until 8:00 p.m. Participation in weekend community outreach events and health fairs is expected, providing valuable opportunities for networking and collaboration within the healthcare landscape.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for individuals and families, ensuring thorough evaluations of their medical histories and current health statuses.
• Diagnose and manage both acute and chronic medical conditions, developing personalized care plans that include preventive care and health promotion strategies tailored to the needs of the patient.
• Collaborate with local community organizations and stakeholders to design and implement effective health education programs that inform and empower our clients.
• Deliver culturally sensitive care to a diverse patient population, advocating for patients' rights and addressing barriers to access, including language and financial barriers.
• Coordinate referrals to specialized care and ensure appropriate follow-up procedures are followed, emphasizing continuity of care.
• Maintain accurate and detailed electronic medical records (EMR) in compliance with healthcare regulations and organizational standards.
• Participate in community outreach events and health fairs, providing health screenings and education that raise awareness about various health issues.
• Stay informed of community health trends, evidence-based practices, and changes in healthcare policies that may impact service delivery.

Patient Population / Client Focus

The Family Nurse Practitioner will primarily serve a diverse clientele, including children, adolescents, and families seeking preventative and acute care. Special attention is given to underserved populations facing barriers to healthcare access. Your role will be instrumental in implementing strategies that strive for health equity and improved community health outcomes.

Qualifications

• A current certification as a Nurse Practitioner.
• An active nursing license valid in the state of New York.
• A Bachelor's degree in nursing; a Master’s degree in nursing is preferred.
• Minimum of 3 years of clinical experience, with a strong preference for experience in community health or public health settings.
• Exceptional interpersonal and communication skills to effectively connect with diverse patient populations and interdisciplinary teams.
• A deep commitment to social justice and health equity, with a passion for advocating for the rights and needs of vulnerable communities.

Preferred Experience

• Experience in pediatric care or family practice is highly desirable.
• Previous involvement in community engagement, outreach programs, or public policy initiatives related to health care.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 per year, commensurate with experience and qualifications.
