Work Environment

Located in Uniondale, New York, this onsite role operates within a home care setting, where you will interact with patients in their own environments. Key Responsibilities

• Perform comprehensive health assessments, engaging in detailed physical examinations to evaluate patient health status.
• Effectively order, perform, and interpret diagnostic testing, utilizing results to inform clinical decision-making.
• Collaborate with a multidisciplinary team to create and implement individualized care plans that are patient-centered and holistic.
• Prescribe medications and deliver timely treatments, ensuring adherence to established guidelines and protocols.

Patient Population / Client Focus

Qualifications

• Active and unrestricted Nurse Practitioner license in New York State.

• A minimum of 2 years' experience as a Nurse Practitioner, particularly in a family medicine context.

Compensation and Benefits
