Work Environment

This role will take place onsite in a home care setting, where you will work closely with a cohesive team consisting of physicians, nurse practitioners, physician assistants, medical assistants, and care managers. We have a dedicated medical scribe that facilitates the documentation process, allowing you to focus more on patient care. You will engage directly with a network of exceptional specialists and hospitals for any necessary acute or specialty care.

Key Responsibilities

• Deliver comprehensive and personalized primary care by conducting office visits for routine check-ups and acute medical issues.
• Administer annual wellness visits and health risk assessments, emphasizing a holistic view of each patient's health.
• Ensure thorough, accurate documentation in compliance with HCC and ICD-10 coding standards during patient interactions.
• Coordinate care with other healthcare providers, specialists, and testing facilities to streamline patient experiences.
• Lead initiatives in population health to ensure eligible patients receive necessary screenings, such as biannual breast cancer screenings for women.
• Participate in phone triage and outreach, addressing patient needs outside of scheduled visits.
• Educate patients about their health conditions, develop tailored care plans, and discuss treatment options.
• Engage in community outreach and promotional activities to raise awareness about our services.
• Conduct home visits as necessary, ensuring that care continues outside of the office setting.
• Collaborate with the Medical Director and operational leadership to align treatment strategies and improve patient outcomes.

Patient Population / Client Focus

As a Nurse Practitioner at Pulse Point, you will primarily focus on adults enrolled in Medicare. This patient population often requires nuanced, longitudinal care, which means building trustworthy relationships is paramount. Your ability to connect with patients and demonstrate empathy will significantly enhance their understanding of their care plans and support their health aspirations.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or equivalent NP certificate program.
• National board certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through recognized bodies like AANP or ANCC.
• Possession of an active state Nurse Practitioner license that is non-probationary.
• Current DEA license to prescribe medications.
• Experience working in primary care settings such as internal medicine, geriatrics, or family medicine.
• Strong understanding of HCC documentation, ICD-10 coding practices, and health risk assessments.
• A team-oriented mindset with a strong desire to collaborate with diverse departments.
• Bilingual proficiency is preferred but not mandatory.

Preferred Experience

• Background in working with geriatric populations and understanding their unique health challenges.
• Experience within a value-based care model will be considered a significant advantage.
• A proven track record of effective patient education and engagement strategies.

Compensation and Benefits

The compensation for this full-time Nurse Practitioner role ranges from USD 180,000 to 200,000 annually, with flexibility based on experience and expertise. In addition to competitive salaries, our benefits include:
