Work Environment

This position operates within a home care setting, allowing the Nurse Practitioner to offer essential care services directly in the patients’ own environments. You will engage with various community stakeholders and participate in local health fairs and outreach programs. The typical work hours are Monday through Friday from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m., and possible participation in weekend events. Key Responsibilities

• Conduct thorough health assessments and screenings to evaluate patient conditions and risks in a home environment.
• Diagnose and manage acute and chronic medical conditions, ensuring preventive care measures and health promotion strategies are effectively implemented.
• Work closely with community organizations to develop and roll out educational programs that aim to increase health literacy and access to care.
• Deliver care that respects cultural differences and promotes health equity, advocating for patients’ rights within the healthcare system.
• Coordinate and facilitate referrals to specialists and ensure comprehensive follow-up care, thus maintaining continuity in the patient’s health management.
• Keep meticulous and confidential medical records that comply with local regulations and standards.
• Represent Care Point at various community outreach events, actively participating in raising awareness about health issues and providing screenings.
• Stay abreast of emerging health trends, guidelines, and legislation that affect community health services.

Patient Population / Client Focus

This role will primarily cater to individuals and families from diverse backgrounds residing in Amityville and surrounding areas. The Nurse Practitioner will address a variety of health concerns, particularly focusing on underserved and vulnerable populations who face barriers to accessing traditional healthcare services.

Qualifications

• Possess current certification as a Nurse Practitioner along with an active nursing license for the State of New York.
• Hold a Master’s degree in nursing from an accredited institution, providing a solid educational foundation for clinical practice.
• Have a minimum of three years of clinical experience, preferably in community health or public health environments.

Compensation and Benefits
