Position Overview

We are seeking a dedicated and compassionate Nurse Practitioner to join our team at Bistro Walk-in Clinic located in Babylon, New York. In this full-time role, you will play a crucial part in providing holistic healthcare services to individuals and families, ensuring that their unique medical needs are met with empathy and expertise.

Work Environment

As part of our onsite team, you will work collaboratively in a vibrant setting aimed at fostering wellness among residents. The clinic operates from 9:00 a.m. to 6:00 p.m., with occasional evening clinics running until 8:00 p.m., amounting to approximately 40 hours a week. Flexibility is essential, as you may also engage in community outreach events or health fairs on weekends, further extending our commitment to community health.

Key Responsibilities

• Perform detailed health assessments and screenings to evaluate the medical conditions of patients, creating tailored care plans for optimal health outcomes.
• Diagnose and manage both acute and chronic health issues, ensuring preventive measures are in place through education and wellness promotion.
• Foster partnerships with local community organizations to develop and implement health education initiatives that address the specific needs of our patient population.
• Provide culturally competent care that respects the diverse backgrounds of our patients and advocates for their access to necessary healthcare services.
• Coordinate referrals and facilitate follow-up care to guarantee continuity and quality of care for all patients.
• Maintain accurate and comprehensive medical records that comply with health regulations, ensuring patient confidentiality and information integrity.
• Take an active role in community outreach events, health fairs, and educational sessions aimed at raising awareness about preventive health and available services.
• Stay informed on community health trends, best practices, and relevant healthcare policies that may affect patient care and the broader community.

Patient Population / Client Focus

Your primary focus will be on a diverse population that includes elderly residents of the assisted living facility, as well as individuals and families from the surrounding community. This role requires a keen understanding of the healthcare challenges faced by these groups, along with a commitment to improving their overall health and well-being.

Qualifications

• A valid Nurse Practitioner certification and an active state nursing license.
• A Master’s degree in nursing from an accredited institution.
• A solid foundation of clinical experience with a minimum of 3-5 years, preferably in community health or public health contexts.
• Excellent interpersonal and communication skills, enabling effective engagement with diverse populations.
• A strong commitment to social justice, health equity, and advocacy for vulnerable and underserved communities.

Preferred Experience

Experience in collaborative environments, especially in community health initiatives or organizational partnerships, will be an advantage. Familiarity with electronic health records (EHR) systems and a proactive approach to community engagement are beneficial for success in this role.
