The Happy Haven Health Center is seeking a dedicated Nurse Practitioner (NP) to join our team in Babylon, New York. Work Environment

This role will be housed within our assisted living facility, where you'll work alongside a compassionate team of healthcare professionals. You will enjoy a structured schedule, typically from 9:00 a.m. to 6:00 p.m., with occasional evening clinics running until 8:00 p.m. Community outreach is central to our mission, and you may participate in health fairs and outreach events on weekends. An average workweek will typically total around 40 hours.

Key Responsibilities

• Conduct thorough health assessments and screenings for residents and community members, ensuring all healthcare needs are met.
• Diagnose and manage both acute and chronic medical conditions, emphasizing preventive care and health promotion through tailored treatment plans.
• Collaborate with local health organizations and community stakeholders to develop and implement impactful health education programs that address specific community needs.
• Provide culturally sensitive care, advocating for patients' rights and equitable access to healthcare services without barriers.
• Coordinate patient referrals and follow-up care, ensuring continuity in healthcare delivery and maximizing positive patient outcomes.
• Maintain precise and comprehensive medical records in compliance with all regulatory standards, ensuring confidentiality and accuracy.
• Engage actively in community outreach initiatives, including educational workshops and health fairs, to promote wellness and provide necessary screenings.
• Stay informed on community health trends, evidence-based practices, and relevant healthcare policies to improve service delivery and patient care.

Patient Population / Client Focus

As a Nurse Practitioner at Happy Haven Health Center, you will predominantly serve a diverse patient population, including seniors and underserved community members. Qualifications

• Current certification as a Nurse Practitioner and an active state nursing license.
• A Master’s degree in nursing from an accredited program is required.
• A minimum of three years of clinical experience, focusing on community health or public health settings.
• Demonstrated strong interpersonal and communication skills, with a proven ability to work effectively across diverse populations.

Preferred Experience

• Experience in an assisted living facility or similar healthcare environment is highly desirable.

Compensation and Benefits

Happy Haven Health Center offers a competitive salary in the range of USD 180,000 to 200,000 per year, commensurate with experience and qualifications.
