Work Environment

This position is ideally located onsite at our Islip facility, promoting a collaborative and vibrant atmosphere where health professionals work side by side to deliver exceptional care. The hours of operation are generally from 9:00 a.m. to 6:00 p.m., with the possibility of extending into evening clinics until 8:00 p.m. on occasion. Participation in community outreach events and health fairs may also occur during weekends, further enhancing your involvement and impact within the community.

Key Responsibilities

• Conduct thorough health assessments and screenings tailored to individuals and families, actively identifying their unique health needs.
• Diagnose and manage both acute and chronic conditions, creating personalized care plans that include preventive care and collaborative health promotion strategies.
• Forge partnerships with local organizations to develop health education programs, focusing on preventative health practices and community wellness initiatives.
• Advocate for culturally sensitive care, ensuring that each patient’s rights are upheld and that they have access to necessary healthcare services.
• Manage care continuity by coordinating referrals and follow-up care, enabling seamless patient experiences across the healthcare spectrum.
• Maintain comprehensive and accurate medical records that comply with healthcare regulations and standards.
• Actively participate in community health events, providing valuable health screenings and promoting health awareness among the population.
• Stay informed about the latest health trends, evidence-based practices, and policy changes that affect community health outcomes.

Patient Population / Client Focus

This role primarily focuses on underserved and vulnerable populations within the Islip community. As a Nurse Practitioner in our concierge medicine model, you will address a diverse range of healthcare needs, building long-term relationships with patients and their families. Your work will directly contribute to enhancing the health standards and overall well-being of individuals who may face barriers to traditional healthcare access.

Qualifications

• Current certification as a Nurse Practitioner, along with an active state nursing license in New York.
• A Master's degree in nursing from an accredited institution.
• A minimum of 3-5 years of clinical experience, preferably within community health or public health sectors.
• Strong interpersonal and communication skills, with an innate ability to connect effectively with diverse populations.

Preferred Experience

• Experience in community outreach initiatives or public health practice is highly desirable.

Compensation and Benefits

We offer a competitive salary range of USD 180,000 – 200,000 per year, commensurate with experience and qualifications.
