Work Environment

This role is primarily based in the Islip area, requiring in-person patient interactions in a home care setting that fosters personalized healthcare. The work environment is dynamic and supportive, where collaboration among healthcare professionals is essential. While the schedule typically falls between 9:00 a.m. and 6:00 p.m., occasional evening clinics and participation in weekend community outreach events may be required. This flexible approach allows team members to adapt to the health needs of the community.

Key Responsibilities

• Perform thorough health assessments, screenings, and evaluations for patients of all ages, ensuring personalized care tailored to their needs.
• Diagnose and manage a range of acute and chronic illnesses, promoting preventive care strategies and health education.
• Build collaborative relationships with community organizations to design and implement effective health education programs that address prevailing health disparities.
• Ensure culturally sensitive patient care, advocating for diverse populations to access necessary healthcare services and resources.
• Facilitate patient referrals and coordinate follow-up care to maintain continuity, enhancing positive health outcomes.
• Maintain accurate and detailed patient records in accordance with federal regulations, utilizing our electronic health record (EHR) system efficiently.
• Engage in community outreach initiatives and health fairs, providing essential screenings and consultation to promote health awareness.
• Continuously update knowledge on community health trends, evidence-based practices, and public health policies to inform care delivery.

Qualifications

• Current certification as a Nurse Practitioner is mandatory.
• An active nursing license in the state of New York is required.
• A Master’s degree in nursing from an accredited institution is essential.
• Proven clinical experience of 3-5 years, with a strong preference for experience in community health or public health environments.
• Exceptional interpersonal and communication skills, facilitating effective collaboration with diverse patient populations and interdisciplinary teams.

Preferred Experience

Compensation and Benefits
