Work Environment

This role is located onsite at our assisted living facility, where you will join a dedicated and collaborative care team made up of physicians, nurse practitioners, physician assistants, medical assistants, registered nurses, care managers, and a dedicated medical scribe. You will have the support of a multidisciplinary team, fostering an integrated approach to patient care that allows for improved outcomes and a more satisfying work experience. Key Responsibilities

As a Nurse Practitioner at Brightwood, you will:

• Deliver exceptional primary care to patients, focusing on preventive health measures and chronic disease management.
• Conduct thorough office visits to assess and manage both routine and acute health issues.
• Administer annual wellness visits and health risk assessments, ensuring comprehensive documentation and patient education.
• Coordinate care among various healthcare providers, specialists, testing facilities, and community agencies to optimize patient journeys through the healthcare system.
• Lead initiatives in population health, such as ensuring that eligible patients receive timely and evidence-based screenings like breast cancer assessments.
• Assist in phone triage and outreach efforts to enhance patient communication and engagement.
• Educate and empower patients regarding their health conditions, treatment plans, and self-management techniques.
• Participate in community outreach and promotional activities to enhance public awareness of our services.
• Occasionally conduct home visits for patients with mobility challenges or complex care needs.
• Report directly to the Medical Director, collaborating closely with operational leadership to ensure quality care standards are upheld.

Patient Population / Client Focus

You will primarily engage with older adults residing in the assisted living facility, many of whom have multifaceted health needs. Your work will impact their daily lives, with a primary focus on improving health outcomes through education, preventive care, and chronic disease management. Your ability to build rapport and trust will be essential in helping these patients understand and adhere to their care plans, thus fostering a path to healthier living.

Qualifications

The successful candidate will possess:

• A Master of Science in Nursing, Doctor of Nursing Practice, or completion of an accredited Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• An active, non-probationary state Nurse Practitioner license and an active DEA license.
• A demonstrated background in primary care disciplines such as internal medicine, geriatrics, or family medicine.

Preferred Experience

Additional qualifications that would be beneficial include:

• A solid understanding of HCC (Hierarchical Condition Categories) documentation, ICD-10 coding, and effective health risk assessment strategies.
• A strong passion for teamwork, showcasing the ability to work collaboratively across various departments.
• Experience in population health initiatives focused on improving clinical outcomes.
• Bilingual proficiency in applicable languages, enhancing communication with our diverse patient base.

Compensation and Benefits

This role offers a competitive annual salary ranging from $115,000 to $150,000, commensurate with experience and expertise. In addition, we provide a comprehensive benefits package that includes:
