We are seeking a dedicated and compassionate full-time Nurse Practitioner specializing in Adult-Gerontology to join our esteemed team at Center Point Healthcare Solution Management Practice in Islip, New York. As a vital member of our healthcare team, you will have the opportunity to provide personalized, high-quality primary care to adults, particularly those covered by Medicare. This role offers a competitive salary range of USD 120,000 – 160,000 yearly, depending on experience and qualifications.

Work Environment

This role is onsite, providing care within our outpatient practices and in the community through house calls. Our work setting is uniquely tailored to serve the needs of older adults. You will collaborate closely with a dedicated care team, including primary care physicians, physician assistants, medical assistants, and care managers, ensuring comprehensive support for our patients. The environment is dynamic and collaborative, focused on making a meaningful impact on the health outcomes of our patient population.

Key Responsibilities

• Deliver exceptional primary care services, including management of both routine and acute health issues.
• Conduct thorough annual wellness visits and health risk assessments, ensuring meticulous documentation and a holistic view of each patient’s health.
• Engage in care coordination with other healthcare providers, specialists, testing facilities, and agencies to ensure seamless patient care.
• Lead population health initiatives by promoting preventive screenings and interventions, such as evidence-based breast cancer screenings.
• Assist the care team with phone triage and outreach, ensuring that patient needs are promptly addressed.
• Educate patients about their health conditions, care plans, and recommended treatments, fostering health literacy and engagement.
• Participate in promotional and community outreach activities to spread awareness of our services.
• Perform home visits to patients as necessary, addressing their care needs in a familiar and comfortable setting.
• Report to the Medical Director while working closely with operational leadership for continuous improvement in care delivery.

Patient Population / Client Focus

Your primary focus will be on older adults, particularly those who are Medicare recipients. Our model prioritizes quality over quantity, emphasizing personalized care plans that resonate with our patients’ lifestyles and preferences. This role is essential in helping patients manage chronic conditions, maintain their health, and enhance their quality of life.

Qualifications

• Master of Science in Nursing, Doctor of Nursing Practice, or completion of a Nurse Practitioner Certificate Program.
• National certification as a Family, Adult, or Adult-Gerontology Primary Care Nurse Practitioner through AANP or ANCC.
• Current, non-probationary state Nurse Practitioner license and an active DEA license.
• Experience in primary care settings, particularly in internal medicine, geriatrics, or family medicine.
• Proficient understanding of HCC documentation, ICD-10 coding, and Health Risk Assessments (HRAs).
• A passion for teamwork and collaboration in a cross-functional healthcare environment.

Preferred Experience

• Previous experience in a concierge medicine or house call practice.
• Familiarity with electronic health records (EHR) and practice management software.
• Demonstrated ability to drive population health initiatives and improve patient outcomes.
