Community Health Nurse Practitioner - Full-Time Position

Position Overview

The US Veterans Health Care Clinic is seeking a dedicated and community-oriented Nurse Practitioner to join our team in Islip, New York. Key Responsibilities

• Collaborate closely with community-based organizations and stakeholders to design and implement tailored health education programs aimed at promoting wellness and prevention.
• Conduct comprehensive assessments and provide direct patient care, addressing a wide range of health concerns while ensuring adherence to best practices.
• Advocate vigorously for patients' rights, ensuring access to necessary healthcare services and support within the community.
• Maintain accurate and thorough medical documentation, ensuring compliance with all regulatory standards and providing continuity of care.
• Participate in various community outreach efforts, including health fairs and screening events, to educate the community and raise awareness of available resources.

Patient Population / Client Focus

Your primary focus will be on serving veterans and their families within the Islip community. This unique patient population may exhibit a range of health issues stemming from military service, mental health concerns, and access barriers. As a Nurse Practitioner, you will be instrumental in addressing these challenges, utilizing your skills to provide holistic care that encompasses not just physical health, but also mental and emotional wellbeing.

Qualifications

• A Bachelor's degree in Nursing from an accredited program is required.
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) by either the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Possession of a valid DEA and NPI number is mandatory.
• Strong clinical assessment skills, alongside excellent interpersonal and communication abilities, are essential for building rapport with patients from diverse backgrounds.

Preferred Experience

• A minimum of 2 years of clinical experience, with a preference for experience in community health or public health settings, is desired.
• Proficiency in managing chronic conditions and utilizing evidence-based guidelines in patient care.
• Experience in multidisciplinary collaboration and community engagement initiatives is a plus.

Compensation and Benefits

The annual salary for this position ranges from USD 150,000 to 160,000, commensurate with experience and qualifications.
