Work Environment

This position is primarily onsite at our outpatient office, where you will work in a collaborative and supportive atmosphere alongside a team of healthcare professionals. Your standard working hours will be Monday to Friday, 9 AM to 5 PM, allowing for a structured workweek. Key Responsibilities

• Conduct comprehensive health assessments and screenings for patients, identifying physical, emotional, and social determinants of health that may affect individual care.
• Diagnose, manage, and treat acute and chronic medical conditions while developing personalized care plans for each patient.
• Provide preventive care and health promotion guidance tailored to the community's specific health needs.
• Actively collaborate with local community organizations to design and implement health education programs, thus fostering an informed patient population.
• Utilize cultural competence to provide sensitive care to diverse individuals, ensuring advocacy for their rights and access to essential healthcare services.
• Coordinate referrals and follow-up care, ensuring seamless transitions and continuity of care for patients.
• Maintain accurate and detailed medical records following regulatory standards to ensure compliance and quality standards.
• Participate in community outreach efforts, including health fairs and educational workshops, aimed at promoting wellness and health awareness within the Islip community.
• Stay informed on current health trends, evidence-based practices, and local health policies that can affect care delivery.

Patient Population / Client Focus

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited program.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) through the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• DEA# and NPI# are mandatory for this position.
• Proficiency in electronic health records (EHR) systems is essential for efficient documentation and record-keeping.

Preferred Experience

• A minimum of 2 to 5 years of clinical experience, particularly in primary care or community health settings, is highly preferred.
• Demonstrated strong clinical assessment and diagnostic skills, with a proven ability to work both independently and collaboratively within a healthcare team.
• Excellent communication and interpersonal skills, enabling effective interactions with patients, families, and colleagues.

Compensation and Benefits

We offer a competitive salary ranging from $150,000 to $160,000 annually, commensurate with experience and qualifications.
