Work Environment

Your primary workplace will be an Assisted Living Facility in Smithtown, NY, where you will interact closely with residents, their families, and interdisciplinary teams. This onsite setting allows for comprehensive and personalized care delivery within the community context. You will work approximately 40 hours per week, with a standard schedule from 9:00 a.m. to 6:00 p.m., and occasional evening clinics until 8:00 p.m. Weekend participation in community outreach events or health fairs may also be required.

Key Responsibilities

As a Nurse Practitioner at Eastside Internal Medicine, you will assume a range of responsibilities including:

• Conducting comprehensive health assessments and screenings for patients in the Assisted Living Facility and broader community.
• Diagnosing and managing both acute and chronic medical conditions, providing preventive care and health promotion initiatives.
• Collaborating with local community organizations to develop tailored health education programs that address prevalent health issues.
• Delivering culturally sensitive care, advocating for patients’ rights, and ensuring equitable access to necessary healthcare services.
• Coordinating patient referrals and follow-up care to maintain continuity in treatment plans.
• Maintaining precise and accurate medical records in accordance with regulatory standards and institutional protocols.

Patient Population / Client Focus

Your practice will primarily focus on individuals residing in our Assisted Living Facility, which consists of a diverse patient population with varying health care needs, including chronic disease management, preventive care, and specialized health education. You will play a pivotal role in addressing the unique needs of these residents, ensuring they receive comprehensive care tailored to their individual circumstances.

Qualifications

The ideal candidate will possess the following qualifications:

• Current certification as a Nurse Practitioner with a strong background in Family Medicine.
• An active and valid state nursing license in New York.
• A Master’s degree in nursing from an accredited institution.
• Proven proficiency in delivering primary and preventive health care.
