Full-Time Community-Focused Nurse Practitioner - Concierge Medicine

Happy Haven Health Center is actively seeking a dedicated and compassionate Nurse Practitioner (NP) to join our team in West Islip, New York. Work Environment

This position is primarily onsite, where you will interact directly with patients in their homes and within the community. Typical work hours will be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Participation in community outreach events and health fairs during weekends may also be required. An average workweek will total approximately 40 hours, providing a balanced structure to your professional life.

Key Responsibilities

• Conduct thorough health assessments and screenings for individuals and families, adapting your approach based on each patient's unique needs.
• Diagnose and manage both acute and chronic medical conditions, providing preventive care and promoting health education tailored to the community.
• Engage and collaborate with local organizations and stakeholders to design and implement impactful health education initiatives.
• Advocate for patients by ensuring their rights are upheld and providing culturally appropriate care to promote health equity.
• Coordinate referrals and follow-up care, ensuring seamless communication and continuity for all patients.
• Maintain meticulous and accurate medical records, complying with all regulatory standards and best practices.
• Participate actively in community events and health fairs, providing valuable health screenings and raising awareness of local health resources.
• Continuously update your knowledge of community health trends, evidence-based practices, and pertinent policies affecting the population you serve.

Qualifications

• Active certification as a Nurse Practitioner and a current state nursing license.
• A Master's degree in nursing from an accredited nursing program is required.
• A minimum of three years of clinical experience, ideally within community health or public health contexts, is essential.
• Demonstrated strong interpersonal and communication skills, with a proven ability to engage effectively with diverse populations.
• A commitment to social justice and health equity, with a passion for advocating for underserved communities.

Preferred Experience

• Experience in concierge medicine or house-call models is highly preferred.
• Background in community organizing or working with nonprofit health organizations can be beneficial.
• Familiarity with electronic health record (EHR) systems for maintaining patient documentation.

Compensation and Benefits

Happy Haven Health Center offers a competitive salary range of $180,000 to $200,000 annually, commensurate with your experience and qualifications.
