Work Environment

The position is based onsite, where you will primarily engage with patients in a home care environment. You will work closely with a multidisciplinary team and contribute to a collaborative atmosphere that values communication, respect, and a patient-centered approach. Our clinic’s operational hours are primarily from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in community outreach during weekends may also be required. This averages out to approximately 40 hours of work per week.

Key Responsibilities

• Conduct thorough health assessments and screenings for a diverse clientele, ensuring individualized care plans are developed and implemented.
• Diagnose and manage both acute and chronic medical conditions, integrating preventive care and health promotion strategies into practice.
• Collaborate with community organizations and stakeholders to design and execute targeted health education programs aimed at improving health literacy and public health awareness.
• Provide culturally competent care that respects the values and preferences of patients while advocating for their rights and access to necessary healthcare resources.
• Coordinate referrals and follow-up care, ensuring continuity across healthcare services for patients.
• Maintain meticulous and compliant medical records consistent with regulatory standards, safeguarding patient confidentiality and data integrity.
• Engage actively in community outreach initiatives and health fairs to deliver health screenings, raise awareness, and foster relationships with local populations.
• Stay informed about emerging community health trends and implement evidence-based practices to enhance the quality of care.

Qualifications

• Current certification as a Nurse Practitioner.
• Active nursing license in New York State.
• A Master's degree in Nursing from an accredited program is required.

Compensation and Benefits

Care Point Nurse Clinic offers a competitive salary range of $180,000 to $200,000 per year, commensurate with your experience and qualifications.
