Work Environment

This position is based onsite in a school setting, allowing you to be directly involved in the health promotion of students and their families. The work schedule typically runs from 9:00 a.m. to 6:00 p.m., with some evening clinics extending until 8:00 p.m. Occasional participation in community outreach events or health fairs on weekends may be expected. Key Responsibilities

• Conduct comprehensive health assessments and screenings for students and their families to identify health needs and provide appropriate interventions.
• Diagnose and manage a variety of acute and chronic medical conditions, developing individualized care plans that include preventive measures and health promotion activities.
• Collaborate with public health organizations and community stakeholders to create and implement health education programs tailored to the needs of the community.
• Advocate for patient rights, ensuring equitable access to healthcare services and culturally sensitive care practices to best serve diverse populations.
• Handle coordination of referrals and manage follow-up care to ensure continuity and consistency in patient treatment and support.
• Maintain detailed and compliant medical records, ensuring accuracy and adherence to healthcare regulations and standards.
• Participate actively in community outreach initiatives, offering health screenings and educational workshops that foster awareness and promote healthy behaviors.
• Stay informed about current public health trends, community health issues, and evidence-based practices that will enhance patient care and organizational effectiveness.

Patient Population / Client Focus

The primary patient population includes students within the school system, as well as their families. You'll work closely with children and adolescents, addressing their unique health concerns while promoting mental, emotional, and physical well-being. Given the school environment, you'll also provide guidance on issues related to growth, development, and preventive care to help foster lifelong health habits in young people.

Qualifications

• Current certification as a Nurse Practitioner and an active state nursing license in New York.
• A Bachelor’s degree in nursing from an accredited institution; a Master’s degree in nursing is strongly preferred.
• A minimum of 3 years of clinical experience, particularly in community health or public health settings.
• Demonstrated strong interpersonal and communication abilities, with a profound commitment to working with diverse communities.
• A passionate advocate for social justice and health equity, especially for vulnerable and marginalized groups.

Preferred Experience

• Previous experience in a school setting or community health clinic is advantageous.

Compensation and Benefits

This position offers a competitive salary range between USD 180,000 and 200,000 annually, commensurate with qualifications and experience.
