Work Environment

This role is based onsite in our outpatient office in Woodmere, New York, a vibrant community that is home to a diverse population. You will be part of a supportive network of healthcare professionals who are committed to delivering exceptional care. The standard work schedule will be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics and community outreach initiatives that may occur on weekends. Expect to dedicate about 40 hours a week to this meaningful work.

Key Responsibilities

• Conduct thorough health assessments and screenings to evaluate the health needs of individuals and families within the community.
• Diagnose and manage both acute and chronic medical conditions, providing preventive care and health promotion strategies.
• Work collaboratively with diverse community organizations and stakeholders to design and implement engaging health education programs aimed at promoting overall community health.
• Deliver culturally sensitive care, ensuring equitable advocacy for patients' rights and providing necessary resources for healthcare access.
• Manage referrals and follow-up care to ensure patients receive consistent and ongoing treatment support.
• Maintain accurate and comprehensive medical records in compliance with healthcare regulations and standards.
• Participate in community events, health fairs, and outreach initiatives to increase health awareness and offer vital health screenings.
• Stay informed about community health trends and evidence-based practices to continuously enhance service quality and patient outcomes.

Patient Population / Client Focus

This role primarily serves underserved populations in Woodmere, responding to the diverse needs of individuals and families facing barriers to healthcare access. Qualifications

• Current certification as a Nurse Practitioner in New York State.
• Active state nursing license.
• A Master’s degree in Nursing from an accredited program.

• A commitment to social justice, health equity, and advocating for vulnerable populations is vital.

Preferred Experience

• A minimum of three years of clinical experience, especially in community health or public health settings, is preferred.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 per year, commensurate with experience and qualifications.
