Work Environment

This onsite position is based in a school setting, where you will engage closely with students, families, and school staff to identify and address health-related issues. The work schedule is structured between 9:00 a.m. and 6:00 p.m., with occasional evening clinics available until 8:00 p.m. Participation in community outreach events and health fairs during weekends is also a requirement. On average, you will be expected to work approximately 40 hours per week in a supportive and collaborative environment.

Key Responsibilities

• Conduct thorough health assessments and screenings that encompass physical examinations, patient health histories, and necessary tests tailored to community health needs.
• Diagnose and manage both acute and chronic medical conditions, offering preventive care and health promotion tailored to patient demographics.
• Develop and nurture collaborations with community organizations to create and implement effective health education programs that promote healthy lifestyles.
• Deliver culturally competent care, ensuring that each patient’s unique background and needs are respected, while actively advocating for their rights in accessing healthcare.
• Coordinate referrals and ensure effective follow-up care, ensuring patients receive continuous support throughout their healthcare journey.
• Maintain accurate electronic health records in compliance with regulatory standards, ensuring the confidentiality and security of patient information.
• Participate in outreach programs and local health fairs, raising awareness and providing services to those in need within the community.
• Stay informed about community health trends and current public health policies to better serve the patient population effectively.

Patient Population / Client Focus

You will be primarily serving a diverse student population and their families within the school environment. This includes addressing both common health issues and chronic conditions while tailoring services to accommodate various cultural and socioeconomic backgrounds. Your understanding of community dynamics will enhance your ability to connect meaningfully with patients, fostering trust and open communication.

Qualifications

• Current certification as a Nurse Practitioner.
• Active nursing license in New York State.
• A Master’s degree in nursing from an accredited educational institution.
• A minimum of three years of clinical experience, ideally within community health or public health settings.
• Exceptional interpersonal and communication skills, allowing for effective engagement with a diverse array of patients and community stakeholders.
• A demonstrated commitment to social justice, health equity, and advocacy for vulnerable populations.

Preferred Experience

• Experience working within school-based health programs or with children and adolescents in a community health context.

Compensation and Benefits
