Key Responsibilities

• Conduct thorough health assessments and screenings for residents and community members to identify health concerns accurately.
• Diagnose and manage both acute and chronic medical conditions, providing preventive care and proactive health promotion strategies.
• Develop collaborative relationships with community organizations to create and implement impactful health education programs and outreach initiatives.
• Deliver culturally competent care, advocating for every patient's rights and access to necessary healthcare resources.
• Coordinate referrals and seamlessly manage follow-up care to maintain continuity and quality of care for patients.
• Document patient interactions and clinical findings meticulously in our electronic health record (EHR) system, adhering to all regulatory guidelines.
• Engage actively in community outreach efforts and health fairs, raising awareness and providing critical health screenings to the public.
• Stay informed about current community health trends, emerging evidence-based practices, and relevant public health policies that impact patient care.

Patient Population / Client Focus

In this role, you will primarily serve an underserved population, including elderly residents in the assisted living facility and various community members seeking care. Your efforts will significantly contribute to their overall health and well-being, allowing you to make a meaningful impact on their lives through personalized care and support.

Qualifications

• Current certification as a Nurse Practitioner in the state of New York.
• Active New York state nursing license with no disciplinary actions.
• A Master's degree in Nursing from an accredited institution is mandatory.
• A minimum of 3 years of clinical experience, ideally in community health, public health, or similar environments.
• Exceptional interpersonal and communication skills, enabling effective collaboration with diverse populations.
• A strong commitment to advocating for social justice and health equity in healthcare access and delivery.

Preferred Experience

• Experience in geriatric care or within assisted living facilities is recognized as an asset.
• Prior involvement with community health initiatives or outreach programs is highly desirable.

Compensation and Benefits

This position offers a competitive salary ranging from USD 180,000 to 200,000 annually, based on experience and qualifications.
