Work Environment

The Nurse Practitioner will work in a school setting, providing essential health services within the local educational community. In this role, you will function as part of a multidisciplinary team, working closely with school personnel, parents, and community partners. Your typical work schedule will be from 9:00 a.m. to 6:00 p.m., with occasional evening clinics extending until 8:00 p.m. Participation in community outreach events and health fairs may also be required on some weekends, providing opportunities for you to engage with the community beyond the clinical walls.

Key Responsibilities

• Conduct thorough health assessments and screenings to evaluate the health of individuals and families, identifying wellness needs and potential risks.
• Diagnose, treat, and manage both acute and chronic health conditions, ensuring comprehensive preventive care and health promotion strategies.
• Work collaboratively with community organizations and stakeholders to design and facilitate health education programs that empower community members with knowledge about health issues.
• Provide culturally sensitive care and advocate strongly for patients’ rights and equitable access to necessary healthcare services.
• Coordinate referrals and follow-up care, ensuring that patients receive consistent and continuous support throughout their healthcare journeys.
• Maintain meticulous and accurate medical records, ensuring compliance with local, state, and federal regulatory standards.
• Engage in community outreach activities and health fairs to raise awareness about health resources and provide vital health screenings.
• Continuously update your knowledge on community health trends, incorporating evidence-based practices and relevant policies into patient care.

Qualifications

• Current Nurse Practitioner certification and an active nursing license in the state of New York.

Compensation and Benefits

Care Point Nurse Clinic Inc. offers a competitive salary range of $180,000 to $200,000 per year, depending on your experience and qualifications.
