Work Environment

This onsite role requires you to work directly within patients' homes, which allows for a personalized approach to healthcare. Your schedule will typically span from 9:00 a.m. to 6:00 p.m., with possibilities for evening clinics until 8:00 p.m. You may also participate in community outreach events and health fairs on weekends, helping raise awareness about important health issues and promoting healthy practices.

Key Responsibilities

• Conduct comprehensive health assessments, including physical examinations and screenings, to identify health issues and ensure effective management of acute and chronic conditions.
• Diagnose medical conditions and develop evidence-based treatment plans, focusing on preventive care and patient education.
• Collaborate with community stakeholders to design and implement health education programs, addressing specific health concerns relevant to our patient population.
• Provide culturally sensitive care that respects the diverse backgrounds of our patients, ensuring advocacy for their rights and access to needed resources.
• Coordinate patient referrals and follow-up care, working closely with specialists and primary care providers to ensure continuity of service delivery.
• Maintain accurate and detailed medical records, adhering to all regulatory standards and best practices in documentation.
• Engage in community outreach efforts, providing health screenings and education at local events, and fostering partnerships with community organizations.

Qualifications

• Current certification as a Nurse Practitioner and an active New York State nursing license.

Compensation and Benefits

We offer a competitive salary range of $180,000 to $200,000 per year, which is commensurate with your experience and qualifications.
