Work Environment

Operating onsite, this position allows you to engage directly with patients in a home care setting. The clinic environment fosters collaboration among healthcare professionals and community members, where you will conduct patient assessments, provide education, and participate in outreach initiatives. The typical work schedule spans from 9:00 a.m. to 6:00 p.m., with the possibility of evening clinics until 8:00 p.m., and occasional weekend outreach commitments to enhance healthcare access for underserved populations.

Key Responsibilities

• Conduct thorough health assessments, screenings, and evaluations for patients, taking into account their unique healthcare needs.
• Diagnose and manage various acute and chronic health conditions, utilizing evidence-based approaches to preventive care and health promotion.
• Collaborate with local organizations to develop, implement, and facilitate health education programs tailored to community needs.
• Provide culturally competent care, advocating for patients' rights and ensuring equitable access to medical services.
• Coordinate referrals and manage follow-up care to ensure continuity in patient care and support.
• Maintain comprehensive and accurate medical records, upholding regulatory compliance and best practices.
• Engage actively in community outreach events and health fairs, delivering screenings and health education to improve community awareness.

Patient Population / Client Focus

Qualifications
