Work Environment

This role is based onsite at our state-of-the-art assisted living facility in Jamaica, NY, where you will engage with residents and their families in a supportive and collaborative environment. The typical work schedule is Monday to Friday, from 9:00 a.m. to 6:00 p.m., with some evening clinics extending until 8:00 p.m. Additionally, you may be involved in community outreach and health fairs during weekends, ensuring a comprehensive approach to public health and wellness. The role averages approximately 40 hours per week.

Key Responsibilities

• Conduct thorough health assessments and screenings to evaluate the wellbeing of individuals and families.
• Diagnose and provide management plans for acute and chronic conditions while promoting preventive care and health favors.
• Work collaboratively with community partners to design and implement targeted health education programs tailored to community needs.
• Deliver culturally competent care and advocate for the rights of patients, ensuring equitable access to healthcare resources.
• Coordinate referrals and follow-up care to guarantee continuity, maintaining strong relationships with external healthcare providers.
• Document medical history and treatment plans in compliance with regulatory standards, employing electronic health record (EHR) systems for accurate record-keeping.
• Engage with the community by participating in outreach events and health fairs, providing education and accessible health screenings to improve population health.
• Stay informed about current health trends, evidence-based practices, and policies that affect the community’s health.

Patient Population / Client Focus

The nurse practitioner will primarily serve residents of the assisted living facility, as well as members of the surrounding community. This demographic includes elderly individuals and those with chronic health issues who may require specialized care and a tailored approach to wellness. You will be responsible for addressing their unique healthcare concerns in a culturally sensitive manner.

Qualifications

• Current certification as a Nurse Practitioner and an active state nursing license.
• A Master’s degree in Nursing from an accredited institution.
• A minimum of 3-5 years of clinical experience, particularly in community health or public health settings.
• Strong interpersonal skills, enabling effective communication across diverse populations.
• A demonstrated commitment to social justice and health equity for underserved communities.

Preferred Experience

Compensation and Benefits
