Position Overview

We are seeking a dedicated and compassionate Community Health Nurse Practitioner to join our team at the US Veterans Health Care Clinic in Long Beach, New York. Key Responsibilities

• Develop and implement comprehensive health education programs in collaboration with community organizations, focusing on preventive care and wellness strategies.
• Conduct thorough assessments, diagnosis, and management of diverse patient health issues, providing culturally sensitive interventions that respect the backgrounds of all individuals.
• Maintain accurate and detailed medical records following regulatory standards, ensuring continuity of care and compliance with quality assurance protocols.
• Participate actively in community outreach events and health fairs, offering screenings, educational workshops, and resources that promote better health practices.
• Stay well-informed about emerging trends in public health, community health developments, and relevant healthcare policies to enhance service delivery.

Patient Population / Client Focus

You will focus primarily on underserved individuals and families who require tailored healthcare solutions. By interacting with a diverse population, you will address health disparities and champion initiatives that empower patients to take charge of their health. The ultimate goal is to provide education and services that lead to improved health outcomes within the community.

Qualifications

• A Master’s or Doctoral degree in Nursing from an accredited institution.
• Certification as a Family Nurse Practitioner (FNP), Adult-Gerontology Nurse Practitioner (ANP), or Adult-Gerontology Primary Care Nurse Practitioner (AGNP) through recognized accrediting bodies such as the American Nurses Credentialing Center (ANCC) or the American Academy of Nurse Practitioners (AANP).
• Current, unrestricted Nurse Practitioner (APRN) license in New York State.
• Valid DEA and NPI numbers are required.
• Strong clinical assessment skills, with the ability to conduct comprehensive evaluations and develop effective treatment plans.

Preferred Experience

• A minimum of 2 years of clinical experience, specifically within community health or public health settings, is preferred.
• Exceptional interpersonal and communication abilities, enabling effective engagement with patients from varied backgrounds.
• Experience in implementing health education programs and conducting community outreach initiatives, enhancing health literacy among populations.

Compensation and Benefits
