Work Environment

Typical working hours range from 9:00 a.m. to 6:00 p.m., with some flexibility required for evening clinics until 8:00 p.m. Weekend participation in health fairs and community outreach activities will also be expected.

Key Responsibilities

• Conduct thorough health assessments including mental health evaluations, identifying both acute and chronic conditions among patients.
• Develop and implement treatment plans that emphasize mental health promotion, preventive care, and disease management tailored to individual patient needs.
• Actively coordinate and manage referrals to specialists, ensuring seamless care and follow-up for all patients.
• Engage with community organizations to design and launch health education initiatives that empower individuals to take charge of their health.
• Maintain meticulous medical records, ensuring compliance with federal and state healthcare regulations and organizational policies.
• Participate regularly in community health initiatives—offering screenings, educational workshops, and promoting awareness about mental health resources.

Patient Population / Client Focus

The ideal candidate will work closely with a diverse patient population, including individuals and families from various cultural backgrounds. Qualifications

• Current Nurse Practitioner certification and active state nursing license in New York.
• Master's degree in Nursing from an accredited institution.
• A minimum of 3 years of clinical experience, with a strong preference for experience in community health or public health settings.

Preferred Experience

• Experience working directly with mental health patients, specifically in a primary care or community health capacity.

Compensation and Benefits
