Work Environment

You will be working in a vibrant assisted living facility, where the emphasis is placed on interdisciplinary collaboration and patient-centered approaches. The work schedule typically runs from 9:00 a.m. to 6:00 p.m., with occasional evening clinics until 8:00 p.m. Participation in community outreach events or health fairs on weekends may be necessary to advance our health promotion efforts. Key Responsibilities

• Conduct Comprehensive Health Assessments: You will perform thorough health evaluations and screenings for patients, gathering detailed medical histories and relevant physical assessments to inform diagnosis and treatment plans.
• Manage Patient Care: Diagnose and manage a variety of acute and chronic medical conditions, ensuring that each patient receives appropriate preventive care and health promotion strategies tailored to their needs.
• Health Education Implementation: Collaborate with local organizations to devise and facilitate targeted health education programs that address prevalent health issues within the community.
• Patient Advocacy: Provide culturally sensitive care and advocate for patients’ rights, working diligently to ensure they have access to the necessary healthcare services.

Qualifications

• Current certification as a Nurse Practitioner.
• Active nursing license within the state of New York.
