Work Environment

This position is based on-site at a local school, fostering an environment where you can directly engage with children and families while contributing to their overall health and education. You will work collaboratively with other healthcare professionals and school staff to ensure that students have access to necessary health services. The average workweek consists of approximately 40 hours, with a schedule that ranges from 9:00 a.m. to 6:00 p.m. and may include occasional evening clinics till 8:00 p.m. Additionally, participation in community outreach events or health fairs on weekends may be part of your role.

Key Responsibilities

• Conduct comprehensive health assessments and screenings for individuals and families within the school and greater community, identifying both immediate and preventive health needs.
• Diagnose and manage acute and chronic medical conditions, including the development of individualized care plans focused on health promotion and disease prevention.
• Collaborate with community organizations, educational stakeholders, and healthcare professionals to create and implement targeted health education programs that empower patients.
• Provide culturally sensitive care, advocating for patient rights and ensuring that all individuals have access to necessary healthcare resources.
• Coordinate referrals and follow-up care to guarantee the continuity of care and maximize health outcomes for each patient.

Qualifications

Compensation and Benefits
